Become A Patient

Salvus Center, Inc., operates like a primary care doctor’s office. We offer examinations, imaging procedures, and laboratory tests. The staff includes two (2) Supervising Physicians, two (2) full-time nurse practitioners, two (2) LPNs and one (1) MA/Interpreter. We also work with specialty physicians who provide services to our patients in the clinic and in their offices. The clinic is open during hours that are convenient to most people’s schedules.

and fax it and the required materials to (615) 451-0121 Att: Intake Coordinator. Someone from Salvus Center will contact you to schedule your first appointment.

You’re an applicable patient if…

You work and do not have medical insurance

You live in Sumner County

You live without medical insurance in a qualifying household

Download the New Patient Application, fill it out
and fax to (615) 451-0121 Att: Intake Coordinator

Make an Appointment

Your application and materials will be reviewed and someone
from Salvus Center will contact you to schedule your first appointment.

At the Appointment

Come to the office on the day of your scheduled appointment 15 minutes early.
Amount due ranges based on scale via cash or check as we do not accept credit cards. If needed, please bring a translator.

Make sure you have:

  • A tax return (or fax this beforehand)
  • At least 3 pay stubs • Mail (proof of Sumner County residence)
  • Download and fill out paperwork (or arrive early and fill it out at the appointment)
